Here are some forms that you may need to complete your driver's education and take a road test.

The following forms are available to download. Please note these forms require Adobe Reader. If you do not have Adobe Reader installed you may download it free at Adobe.com

Driver's Manual
Revised September 2011. The manual is also available in book form at the RMV for $5.

Permit Application ($30)
Fill this form out and bring it with you to a full-service RMV branch. You will need to pass a test to earn your permit. If you are under 18 years of age, the Parental Consent For Minor section must be completed by a parent/guardian.

License Application ($50)
Fill this form out and bring it with you to a full-service RMV branch. You will need to pass a road test to earn your license. If you are under 18 years of age, the Parental Consent For Minor section must be completed by a parent/guardian.

Road Test Application ($20)
Fill this form out and bring it with you to the road test site. If you are under 18 years of age, the Parental Consent For Minor section must be completed by a parent/guardian. If you take a road test with our driving school, this form will be provided to you.

Supervised Driving Log
It is a requirement of the RMV that all JOL students complete at least an additional 40 hours of supervised, behind-the-wheel driving with a parent/guardian. This form is used to log those hours and will be handed out at the parent classes at Becky's Driving School. If you would like to receive one sooner to start practicing, please feel free to download one here.

 

The following forms are specific to Becky's Driving School, and are not required by the RMV.

Contract
Please review and sign this contract. If you are under 18 years of age, you will need to have a parent/guardian sign as well. Please either bring it in with your deposit, or on the first day of class.

Student Ability Sheet
This form is required by all Becky's Driving School students for every road lesson. You will receive one on the first day of class. If you lose it, you can download a new one and make sure to bring it with you on your next lesson.
